Understanding Your Policy
Premium & Subsidy
In the world of ACA, your monthly premium is the amount you pay but may not be the total amount of the policy. Premium - Subsidy = Your cost.
Deductible
The amount you pay out-of-pocket for specific covered services before insurance kicks in. Example: $1,000 deductible could mean if you got to the ER, the first $1,000 of the bill is your responsibility.
Copayment (Copay)
A fixed fee you must pay for specific services. Example: $25 copay for primary care visits OR $10 copay for generic drug prescriptions.
Coinsurance
Similar to a deductible but typically on a percentage basis. This represents "your share" of expenses. Example: 20% for imaging AFTER your deductible.
Out-of-Pocket Maximum (OPM)
The maximum amount you will ever pay during a policy year. After this limit is reached all, 100%, of your medical expenses will be paid by insurance.
Network
The facility, providers or suppliers that your health insurance carrier has contracted with to provide services.
